245D SERVICES
3H CARE is a 245D licensed provider of Homemaker, Respite, Chores, Individual Home Supports (IHS) without training, IHS with training, and Night supervision services.
To support the health, and independence of those we serve, 3H CARE offers 245D services to people with disabilities. These services can provide the level of assistance, supervision and care that is necessary to ensure the health and safety of your loved ones Read the program descriptions below to find out how 3H CARE is helping Minnesota people live better lives
Night Supervision is a 245D service comprising of overnight assistance and monitoring provided by a staff in the client’s home when the client has an assessed need in one of the following covered service areas:
- Assistance with skill development bolsters
- Instrumental activities of daily living (IADL)
- Activities of daily living (ADLs)
Homemaker Services extend from simple household cleaning and laundry to assistance with home management and activities of daily living. Homemaker includes:
- Light housekeeping tasks
- Laundry services
- Meal preparation
- Shopping
- Arranging for transportation
Individualized Home Supports with training provides support, assistance and observation to adults or minors who live in their own homes. A caregiver provides direct supervision, guidance, instruction, and assistance with activities of daily living, or assistance with planning and going to community activities. Individualized Home Supports Without Training services can be provided in the person’s own home or community.
Respite –
Respite is a short-term care service planned to provide a break to a client’s essential caregiver. Respite can be provided in the client’s home or the respite caregiver’s home.
Chores-
Chore services: Caregiver provides a service to a client in their home to help maintain a clean, sanitary and safe environment.
