Founded and registered in Minnesota in 2012, 3H Care began providing services in 2013. Today, we support individuals and families throughout Minnesota with CFSS Agency Model, 245D and Chore Services.
Our story
More than a decade of supporting life at home and in the community.
3H Care was founded and registered in Minnesota in 2012, with services beginning in 2013. From the beginning, our focus has been practical: help people receive dependable support while preserving dignity, choice and connection to the people and communities that matter to them.
As our organization has grown, that focus has stayed the same. We work with individuals, families, caregivers, case managers and referral partners across Minnesota to coordinate authorized services thoughtfully and respectfully.
