Common questions

245D FAQs

What does “245D” mean?

Chapter 245D is Minnesota’s licensing framework for certain Home and Community-Based Services. These services are intended to support health, safety, dignity and independence in home and community settings.

How do I know which 245D service is right for me?

The right service depends on the person’s assessed needs, goals, living situation and program eligibility. A case manager or lead agency generally helps identify and authorize appropriate waiver services.

Does 3H Care determine eligibility?

No. Eligibility and service authorization are determined through the applicable public program and lead agency. 3H Care can explain the services we provide and coordinate with the person’s care team once a referral is made.

Do you serve areas outside the Twin Cities?

Yes. 3H Care serves clients throughout Minnesota. Contact us about the specific service and location so we can review availability.
