CFSS Agency Model

More choice and control over everyday support.

CFSS is a Minnesota program that can support daily living, health-related tasks, observation and redirection, and certain independent-living activities when authorized. 3H Care provides CFSS through the Agency Model only.

Individualized support at home and in the community.

Our 245D services include IHS with training, IHS without training, IHS with family training, night supervision, respite, homemaking, 24-hour emergency assistance and adult companion services.

Chore Services

Practical help to keep the home safer and manageable.

Authorized chore support can help with heavier household or property tasks that are difficult for a person to complete safely and are included in the person’s approved support plan.

Choice, family & familiar support

Someone you already know may be part of your support.

Under Minnesota CFSS, a participant may be able to choose a worker they know, including certain family members, when program and worker requirements are met. 3H Care supports CFSS participants through the Agency Model and serves as the worker’s employer/provider agency.

Not every family situation or role is eligible, and other programs have different requirements. We can explain the 3H Care side of the process and help you understand where to go next.

We complete applicable provider paperwork, worker onboarding, service planning and scheduling requirements.

Once required steps are complete and the service is authorized, approved support can begin.

What clients and families often ask.

Do I need to know which service I need before contacting 3H Care?

No. Tell us what kind of support you are looking for and any program information you already have. We can explain the services 3H Care provides and help identify the appropriate provider-side next step.

Can a family member or someone I know be my paid caregiver?

In CFSS, certain family members and familiar workers may be able to provide paid support when program requirements are met. Rules vary by role and service. See our CFSS and Caregivers pages for more detail.

Does 3H Care determine whether I am eligible?

No. Eligibility and service authorization are handled by the appropriate Minnesota program, lead agency or authorized partner. 3H Care provides services after the applicable program and authorization requirements are satisfied.

Do you serve clients throughout Minnesota?

Yes. 3H Care serves clients across Minnesota, subject to service availability, authorization and our ability to safely staff and coordinate the requested service.

How quickly can services start?

Timing varies based on the program, authorization, required paperwork, staffing, worker onboarding and the specific service plan. We can discuss the steps that apply to your situation.
