CFSS is Minnesota’s person-centered home and community support program. 3H Care helps individuals and families use authorized services to support daily living, independence and participation in the community.
Understanding CFSS
What CFSS can support
CFSS is replacing Minnesota’s PCA and Consumer Support Grant programs. It covers core personal assistance supports while adding more options for how people organize and direct their services.
Independent living tasks
Support with instrumental activities such as meal preparation, shopping, laundry and other tasks that help a person remain independent.
Health-related tasks
Authorized assistance with health-related procedures and tasks, including support with self-administered medications when included in the plan.
Observation & redirection
Support that may include observation and redirection of behavior when it is part of the person’s assessed need and authorized services.
How 3H Care provides CFSS
CFSS Agency Model
3H Care provides CFSS through the Agency Model. We serve as the provider agency and employ the workers who deliver authorized CFSS services, while the participant remains actively involved in directing their care and choosing qualified workers.
3H Care CFSS Agency Model
Professional agency support with meaningful participant choice.
Under the Agency Model, 3H Care manages employer responsibilities for CFSS workers and coordinates services within the participant’s approved service delivery plan and authorization.

Your county, tribal nation or managed care organization determines eligibility and authorization. Your consultation services provider helps develop the CFSS service delivery plan, and 3H Care coordinates the Agency Model services we are authorized to provide.
Care from people you know
Family members may be able to serve as paid workers.
CFSS expands options for who may provide support. Depending on program requirements and the person’s situation, family members — including a spouse or a parent of a minor — may be able to serve as paid support workers.
Workers still must meet applicable enrollment, training, background-study and service-plan requirements.

Request an Assessment
Contact your county, tribal nation or managed care organization, depending on your situation, to request an assessment.
Receive Eligibility Results
Your lead agency explains the assessment results and the CFSS services for which you are eligible.
Choose Consultation Services
Everyone using CFSS selects a separate consultation services provider to learn about CFSS and develop a service delivery plan.
Choose an Agency Model Provider
If you choose the Agency Model, you can select 3H Care as your CFSS provider agency, subject to availability and authorization.
Begin Authorized Services
After the plan and authorization are in place, 3H Care coordinates the services we are approved to provide.

CFSS FAQs
What is CFSS?
Community First Services and Supports is a Minnesota health care program that helps eligible people with disabilities, chronic illnesses and older adults receive support in their homes and communities. It is replacing PCA and the Consumer Support Grant.
Who decides whether I qualify for CFSS?
Your county, tribal nation or managed care organization conducts the assessment and determines eligibility and authorization. 3H Care does not determine eligibility.
Does 3H Care offer the CFSS Budget Model?
No. 3H Care provides CFSS through the Agency Model only. We do not provide the Budget Model or Financial Management Services (FMS).
Can a family member be my paid worker?
CFSS allows more family-member options than the prior PCA program. Certain family members, including a spouse or parent of a minor, may be paid workers when all program requirements are met.
Do I need a consultation services provider?
Yes. Everyone using CFSS selects a consultation services provider. That provider must be separate from the CFSS provider agency or FMS provider used for the person’s services.
Does 3H Care provide services throughout Minnesota?
Yes. 3H Care serves individuals and families across Minnesota, subject to service availability, staffing and authorization.
