Support for adults who need both assistance and skill-building while living in their own home or family home. Training may focus on areas such as community participation, health and safety, household management and adaptive skills.
Designed to build or maintain skills while supporting day-to-day independence.
Everyday assistance
IHS without Training
Support, assistance and supervision for adults or children who need help in one or more community-living areas while living in their own home or family home.
May include support with community participation, health and safety, household management or adaptive skills.
Family-centered training
IHS with Family Training
Support and training for adults or children who live in their family home when the person and/or family needs help in one or more community-living areas.
Training is individualized around the person’s needs and support-plan goals.
Overnight support
Night Supervision
Overnight assistance and monitoring by staff in the person’s own home to address needs identified in the support plan.
A focused option for people who need support or monitoring during overnight hours.
Caregiver relief
Respite Care — In-Home & Out-of-Home
Short-term care when the family member or primary caregiver who normally provides care is absent or needs relief.
3H Care offers both in-home and out-of-home respite when appropriate and authorized.
Home support
Homemaking
Household support that can range from light cleaning to eligible cleaning services with incidental help related to home management and activities of daily living.
The goal is a safer, more manageable home environment based on the person’s authorized service needs.
On-call response
24-Hour Emergency Assistance
On-call counseling, problem-solving and/or immediate response when a person needs assistance at home because of a health or personal emergency.
This service is planned around the person’s identified emergency-support needs.
Connection + supervision
Adult Companion Services
Non-medical care, supervision and socialization for adults age 18 or older. Support may also include verbal cues and incidental help with tasks when they are part of the companion service.
The focus is meaningful support, safety, connection and participation aligned with the person’s support-plan goals.
