Availability depends on the person’s location, authorization, staffing and program requirements. 3H Care does not determine eligibility or approve services; we coordinate the provider side after the appropriate lead agency or care team authorizes services.

What referral partners often ask.
Does 3H Care accept referrals statewide?
Yes. 3H Care serves clients throughout Minnesota, subject to the requested service, authorization, staffing and provider availability in the person’s area.
Can I refer for CFSS Budget Model?
No. 3H Care provides CFSS through the Agency Model only and does not provide the Budget Model or Financial Management Services (FMS).
What 245D services can I refer to 3H Care?
3H Care provides IHS with training, IHS without training, IHS with family training, night supervision, in-home and out-of-home respite, homemaking, 24-hour emergency assistance and adult companion services.
Should I upload records or include client medical details here?
No. This initial inquiry form is intentionally limited. Do not submit protected health information or medical records through this form. We will coordinate an appropriate method for exchanging required referral information.
Does submitting this form guarantee acceptance?
